Sign-In/Sign-Up Sheets

I was wondering how those of you that practice chair massage have your Sign-In/Sign-Up sheets set up?I normally do 15, 20 & 30 minute massages. I have a 2 or 3 minute stretch/clean-up period after each 15 & 20 minute massage & a 5 minute stretch/clean-up period after each 30 minute massage.Thanks in advance for any and all input!

Hey SB-1, welcome to massageplanet.I'm not too sure what information you are after here, but i'll tell you what i do. However be aware that I am a UK therapist so your insurance / legal requirements may be different to mine here.I have a smaller Medical Consent form that my Chair clients fill-in, it consists of personal details (name, address etc), and a series of tick boxes that outline the major contra-indications or potential contra-indications and a "any other information you feel the practitioner should be aware of". Depending on where I am working I have a small supply of these that clients can complete while waiting for their appointment.I can then quickly and easily see anything that may be an issue to the massage and I can discuss any concerns with the client further. I am also very clear that the client understand that the chair work isn't a treatment - its primary purpose is for relaxation. Naturally the client may experience some form of release or therapeutic benefit.Mat xxx

The places that I do chair massage at take care of the sign up sheets themselves. آ One person in the office is in charge of the sheet.One office posts the sheet and the employees sign-up themselves. آ One office basically has standing appointments and they don't really change. آ The contact person calls/emails that day to confirm everyone.If you want, you can make up a form yourself for them to go by. آ At each office, all my appointments are the same length with 5 minute breaks. آ If you are trying to do different lengths of time for different people at the same place this may make it more confusing. آ You might want to consider doing the same length massage for everyone.  It all depends on what the set-up is. آ

I agree with Masthera concerning varied lengths of massage time; keep it simple, sweetie.  If you have your heart set on allowing people more flexibility of time, make it 15 minute increments and they can sign up for 2 time slots.Our form for corporate massage is similar to fozzyo's.  For trade shows or other events, we have a sheet that lists contraindications and has a disclaimer (informed consent), with spaces for the name, address and phone number of about 25 clients [think mailing list].As Masthera said, there is a contact person in the client's office that handles the corporate sign-ups.  Trade shows we have a short waiting line, but we don't sign people up in advance, as folks tend to wander around at these events.

When we budget for an event, the client gets 50 minutes of active time per hour.We group those 10 mins per hour to create 15 minute breaks or 30 minute lunch breaks depending on the length of the event.No one works more than 2.5 hours without a scheduled break. If someone fails to show, they get their break early and have the option to reschedule them during the scheduled break time.
