I am doing a limited number of medical massages and I do prefer a DOCTOR'S NOTE, PRESCRIPTION, or a LETTER OF REFERRAL. The note should state your doctor's name and the referral date. Please bring it with you to the appointment, so I can put it in your file. I don't have a debit/credit machine, but you can do cash/etransfer, and get a receipt for HEALTH INSURANCE SUBMISSION or I direct bill them. I am registered with NHPC and I have been practicing for 8 years. ****heated table is optional **** *I can direct bill MOST health insurance companies. Approximately, 30 or more!
