Charity Event - Consultation Form

    I've been asked to donate my time at an "Alternative Therapists Evening" next month. It's a charity event and there will be 5 or 6 therapists offering different therapies. I'll be giving 15 minute taster sessions (back, neck shoulder massage). I'd like people to fill in a consultation form but feel the one I use for my clients is long and quite detailed. I just wondered if any of you had done these events and what details you asked for in your consultation forms.

    Any ideas would be greatly appreciated. Thanks!

    I have been asked to do taster sessions at a charity event for people with learning diffiuculties, I was also wondering what do with consultation as the one i have for full treatments is too long, would you be willing to share yours with me too please.
    Also any advice on how to run the taster sessions, I am newly qualified so really only used to doing full treatments, I will be doing reiki and reflexology.
